fix: v0.5.1 字符级同行多块变更重复输出修复 - 字符换位专项测试暴露并修复回映射缺陷：同行内多处变更（ne-eq-ne 同行）不再重复输出多个 modified、回拉跨界判定与对齐行判定锚定同一 eq 块避免把全 common 行误判为 added、新增字符换位专项测试组固化对齐任意性下的不变量
